Neprilysin inhibitor
- Natriuretic peptides are not degraded
- Natriuresis, vasodilation and ↓ cardiac remodeling
- ↑ bradykinin → angioedema risk
- Never combine with an ACE inhibitor
- ↑ angiotensin II (also a neprilysin substrate) → paired with an ARB
- Prolongs natriuretic peptides — heart failure benefit
