BOXED WARNING Hyperkalemia — serum potassium can rise ≥5.5 mEq/L; monitor potassium frequently, especially with renal impairment, diabetes, or in elderly/severely ill patients
FDA dosing
Population
Start
Target
Max
Hypertension or edema (in patients who develop hypokalemia on hydrochlorothiazide alone or in whom hypokalemia cannot be risked)
Adults
37.5/25 mg: 1 tablet QD (or initiate here if hypokalemia cannot be risked)
37.5/25 mg one to two QD, or 75/50 mg one QD
Two 37.5/25 mg tablets daily, or one 75/50 mg tablet daily
Renal Contraindicated in anuria and acute/chronic renal insufficiency or significant renal impairment; mild impairment requires frequent electrolyte monitoring
Hepatic Use with caution in impaired hepatic function or progressive liver disease (may precipitate hepatic coma)
Avoid in diabetic patients; monitor electrolytes
Instructions
Give as a single daily dose with monitoring of serum potassium
Avoid divided dosing (increased risk of electrolyte imbalance and renal dysfunction)
Not indicated for initial therapy of edema or hypertension
Available strengths
tab 25/37.5, 50/75 mg
cap 25/37.5 mg
Contraindications
Hyperkalemia — elevated serum potassium levels (greater than or equal to 5.5 mEq/liter)
Concomitant use of other potassium-conserving agents (e.g., spironolactone, amiloride, other triamterene-containing products) or potassium supplementation/potassium-containing salt substitutes/potassium-enriched diets
Anuria, acute and chronic renal insufficiency, or significant renal impairment
Cautions
Hyperkalemia (may be fatal)
Metabolic or respiratory acidosis
Acute myopia and secondary angle-closure glaucoma
Electrolyte imbalance and BUN increases
Hepatic coma risk in liver disease
Renal stones
Folic acid deficiency
Hyperuricemia/gout
Non-melanoma skin cancer (hydrochlorothiazide)
Adverse reactions
Jaundice
Nausea/vomiting
Diarrhea
Dizziness
Headache
Orthostatic hypotension
Hyperkalemia/hypokalemia
Hyponatremia
Elevated BUN/creatinine
Photosensitivity
Rash
Pregnancy Safe use in pregnancy not established; use only if potential benefit justifies risk to fetus (thiazides/triamterene cross placenta)
Lactation Thiazides excreted in human breast milk; triamterene appears in animal milk; if drug essential, stop nursing
